You can come to reception to collect the registration forms, the receptionist will advise you what forms of ID we require. You will then need to return the completed forms along with the appropriate forms of ID.
During the registration process we will advise you of our services & how we operate on the following:
- your registered doctor & who their secretary is
- opening times
- our appointment system
- repeat prescriptions & ways to order
- home visits
- urgent/same day problems
- our patient group (CHAT)
- online services – we can set-up access to our online services at the time of registration.
