1. Online Registration
To register to become a patient at Crown House Surgery you need to live within the practice boundary, please click here
to download a map of the practice boundary to check. If you live within this area then please proceed accordingly following the instructions carefully:
- Click on the SystmOnline logo/link below and you will be redirected to SystmOne's website login page.
- Click the 'register at this practice' button, fill in the form and click submit.
- You will be presented with an online GMS1 form. Please do the following with this form:
- fill it in
- print it out
- post it or hand deliver it to the surgery with one of the following completed New Patient Health Questionnaires:
- 16 years and over
- Under 16 years
- Please then wait while we make contact with you by phone within one working day to arrange a 'New Patient Health Check'.
If you for any reason have been unable to print out either the form or questionnaire then let us know when we phone you and we can post copies to you.

New Patient Health Check
- This is required for all new patients wishing to join the practice.
- We will arrange this appointment when we first make contact with you.
- You will need to bring some form of identification such as a driving license or passport for each individual from your family attending for their
health check.
- If you would like to use the other online services 'Prescriptions & Appointments' then we can issue you with a user name
and password at this appointment. Please remember to ask as this has to be done face to face, it cannot be done online or over the telephone for security and confidentiality reasons.
Please note that this service is not available for under 16 year olds or for their parents on their behalf. Again this is for reasons of confidentiality.
